Is there any way to extract only the audio from a title, instead of having to create the mkv with the video included?

Not with MakeMKV but one could use MKVToolNix to extract the desired audio track to a .mka file.

I have the same question, but with a different goal as OP.
Looks like OP wants to extract the audio track from ripped MKV files, which can be demuxed from the MKV. OTOH I want to rip JUST additional audio tracks from the discs WITHOUT video in order to mux them into my existing rips without re-ripping the whole movie. I found out after ripping about 200+ HD and UHD titles that MakeMKV has been defaulting to the "base" audio tracks rather than the full uncompressed audio. I was trying to rip everything, I didn't realize MakeMKV was doing a minimal rip.
Would it be easier just to re-rip all the movies? At 10-50 minutes per movie, I was hoping to avoid re-ripping my whole library. Maybe I'll just re-rip my UHD movies and leave my HD library. The video is only heavily compressed 1080p anyway, no need for lossless audio!
