OK... so I too bought the Amazon copy (BD + DVD+ UV) and I have the following results:

Judging from what donmelton lists as the proper segment map - it looks like MY copy has a different output file name, and uses the source file name
00795.mpls. The 00778 has the wrong segment maps compared to his, and the output file name of donmelton's is t130, which for my disc has a source file name of 00811.mpls.
Did they really make a ton of different breeds of this disc? It rather ticks me off that I pay money for my movies - retail (Amazon is retail right?
) - and I cannot make an easy backup to play on my media PC. #facepalm
Anyway - I hope the info I provided here helps someone else... I am still ripping my movie, but if the segment maps are what counts - then hopefully I can report back with good news.
EDIT: I also have the same UPC as Havok3114 up above (havok's is from walmart though) UPC - 0 31398 18152 1
