Fixed! You seem to have been correct with the power issues idea. I tried the USB hub built into my monitor as a long shot and, although the console did spit out some AV sync errors, it did produce the MKV files. They play back perfect. Thanks!

No improvement from cleaning and trying various ports and hubs sadly. The Buffalo does appear to have a 5v dc barrel jack which can be used instead of a second USB I am guessing? I could get an adapter for a try. I just bought the disc so I could also return it.

I am using a Buffalo BRUHD-PU3-BK. The disc often does not load in file explorer and I can hear the disc spinning up and down and up and down.
