If I recall correctly these had a non-standard palette, i.e. the transparent colour wasn't palette code 0. +1 for bdsup2sub. If I'm not sure which subtitles I need then I rip them all, extract them all in one command with eac3to and then load them into bdsup2sub to work out which ones I did/didn't need. Then I can go back to MakeMKV if I still have it open and deselect the ones I don't want and rip again...

You can choose a quality profile that will have no (effective) generational loss as you can give it enough bandwidth to be able to encode the MPEG2 blockiness that is already inherent in the files. You could also use something like Handbrake to convert the MPEG2 stream inside the MKV file into something like H.264. It'll still be an MKV but it'll use a codec that the Roku can play.
You will get a generational loss through the conversion though.

Weird. I have both the DVD and the Blu-ray of Black Hawk Down and both ripped with no problems. As one title and with chapters. Clearly there's more than one version or you've found a new bug in MakeMKV.

Renumber the chapters in the second file before the merge and they'll come through intact. It doesn't create duplicate chapter names, which is the problem you're seeing

This ties in with what I've found. I've always had to use eac3to to extract the subtitles direct from the original EVO files in order to be able to process them with bdsup2sub. Once they're in a MakeMKV-produced MKV file they're no use. Fortunately I've ripped all of my HD-DVDs so it's not a big iss...

Popcorn Hours support both VOBSUB and PGS subtitles natively in MKV files, so I don't know what your problem is.
I have hundreds of titles with subtitles in them, in bothh VOBSUB and PGS format, depending on whether they were sourced from DVD or Blu-ray.

I've ripped hundreds of Blu-rays and I can say that, without exception, they always begin at Chapter 00, whereas DVDs always begin at Chapter 01. No, but mkvtoolnix can split at chapters very easily afterwards and it's free.

Interlaced VC-1 is a problem for a lot of players. It's nothing to do with MakeMKV, just with how the video is encoded as it was originally targeted for broadcast.

That's how I rip 95% of my discs to MKV. It works fine for me, and I use the same tool to remove the protection and create the initial ISO for backup purposes.
