Found a solution to anyone that encounters the "title saved 0, failed 1" just downgrade makemkv to version 1.12.2. For some reason the newest version higher will not rip some discs.

So I tried downgrading makemkv to version v1.12.2. And ripping the DVD worked flawlessly. And mkvtoolnix doesn't read ISO images.

The other thing that you might try is opening the .iso backup MakeMKV made with MKVToolNix GUI. I don't know for certain that it will open a DVD backup .iso file, but I think it will. You need to look at the details of WHY it's failing. Odds are, you are experiencing read failures which is almost certainly related to a bad disc, a bad drive, or the combo of the two not working well together. When I rip DVDs/Blu-rays on Makemkv (I'm using latest version) and I choose subtitles the rips /subs end up being forced subtitles. Which I don't want.
