Found a solution to anyone that encounters the "title saved 0, failed 1" just downgrade makemkv to version 1.12.2. For some reason the newest version higher will not rip some discs.

So I tried downgrading makemkv to version v1.12.2. And ripping the DVD worked flawlessly. And mkvtoolnix doesn't read ISO images.

The other thing that you might try is opening the .iso backup MakeMKV made with MKVToolNix GUI. I don't know for certain that it will open a DVD backup .iso file, but I think it will.
