The timing seems to coincide with release of PC software player (PowerDVD) and UHD-BD certified drives. A lot of companies offer lifetime licence and later regret it. New product comes out (UHD BD), people with an existing lifetime license expect to get a free update to handle this product. That's not financially viable for the vendor so they shut down, create a new shell corporation and continue work...

I tried it again a few hours later (ripped some other discs in the meantime) and it worked this time.
Strange, maybe my Bluray drive handles bad discs better when it is hot or something.

I have the same issue, Firefly Disc 2 (UK). Exact same offset as dgodfrey. I guess a bad pressing?

There is usually subs for the commetaries etc. They take up very little space so just grab them all if you are not sure which is which. It is better to have too many subs (you can easily mux them out later when you figure out which is which) then to forget to capture some subs and have to go back to...
