I use the High Profile setting and set the rate factor from 22 to 24, for HD content.
However, the issue you seem to be having is with Handbrake, and not MakeMKV. Therefore, I would recommend that you collect your encode logs, and post at the Handbrake forums.

Contrary to what Criterion has advertised, some of their DTS-HD blurays are PCM. I have not had any problems with ripping these discs, or playing back the rips. For frame x to frame y, I would say that accessing direct from the disc would be best.
However, if you are gonna transcode the whole thing, then go ahead and rip then transcode.
