argh...my issue ended up being my drive quit spinning. Everything works fine for me after the replacement.
I did not get an error/request to grant access to the removable volume after replacing the drive.

I've got the same issue. I tried reinstalling the application and I deleted the permissions in Security & Privacy >> Files and Folders: MakeMKV.app Removable Volumes. Now I don't get the prompts to grant access again. I had to manually add the app to Full Disk Access, but it still does not have ...
