Blu-rays. I've ripped over 300 discs and some of them have had scratches or spots where MakeMKV has to retry reading the section, ultimately failing to do so but it continues ripping anyway. Not necessarily true.I've had discs that gave me debug codes that ultimately successfully ripped with no error message, but were broken/corrupted during playback.

I ripped Spider-Man 2 to an MKV and upon watching in VLC, the audio cut off around the 1:25:00 mark. The video then glitched and skipped ahead to around 1:30:00, and played fine til the end.
