For hosts file did you put a # and some space before that bottom line entry? I tried this and the disk I'm trying to rip still won't decode. It could be that it's not been fixed yet. I was able to get it working, but it takes some effort. These instructions are for Windows. Edit the host file on your computer (C:\Windows\system32\drivers\etc\hosts) and add the following line to the bottom of it.
