I would also suggest getting off 1.03 as it really a crappy firmware. Plenty of people have had read issues using it. The 'dirty' disc actually came as quite a surprise. I handle my discs very carefully. I have ripped many hundreds of discs without any problems (but, never an Ultra HD Blu-ray). Footnote/Update: Cleaning the disc was (most likely) the answer. After cleaning, the disc that failed before worked perfectly. If 1.03 continues to work, I'm going to leave it alone. As they say, if it ain't broke ... :-) Would you like some very picky feedback about MakeMKV, and some less picky fee...
