What I would do in this situation, is make an image of the disk (ignoring errors) and use VLC to play the ISO. That way you have a digital copy is some form. I do this under Linux with ddrescue. It can play fine and still fail in MakeMKV. Certainly seems like the drive is having a problem reading that disk, i.e. hardware error causing MakeMKV to fail.

Sounds Legit. So it could be a dirty lens, misalignment, or any number of issues causing a bad read, but LG just returns the junk with no error. :x
