A corrupt m2ts file, the next version of MakeMKV will have a workaround. For now you can make decrypted backup and play m2ts files. BBC planet earth, 2/4 titles failed
When attempting to save disc1 of BBC's planet earth, 2 of the 4 titles fail to save. Both behave exactly the same -- the rip proceeds normally until the title is 99.9% done, then it says there was an exception, and it failed to save the title. Your error messages look like what I get on Ubuntu when I do not run as root. Try running under sudo and see if that helps.

Is your user a member of cdrom group? Yes -- see output from "groups" command in the above message. Other commands that access the cdrom (like eject, k9copy) work fine, that is why it is so strange. I've had this box for 2+ years now, and never had a problem ripping a dvd (using k9copy). ...
