Have you tried gently cleaning the disc? Those errors show that there's some spots on the disc that the optical drive is having a hard time correctly reading.

The keyDB file needs to go in the folder that's configured as the 'MakeMKV data directory'. You can find what it is set to by looking on the General tab of MakeMKV's preferences.
