If you need a valid private_data.tar file, at the bottom of the first post of the Ultimate UHD Drives Flashing Guide, you'll find a link to one. It is a few years old at this point, but if that's the only thing standing in your way, it might be enough to get started.

MakeMKV doesn't really know anything about the contents of a disc. It just knows what it can from reading the structure of the disc. The _t04 or whatever that is appended on the end is just something MakeMKV does to try to make every title on the disc unique. It appends that incrementing item based ...

Does the problematic disc have AACS v82 encryption? If so, eventually MakeMKV will support it.

If you got another drive that supported LibreDrive, you could play it now.

A drive with the MT1939 platform can't be upgraded to support UHD. You'll need a drive with the MT1959 platform or one of the supported Pioneer drives. Even brand new, fresh-out-of-the-wrapper discs can sometimes benefit from a cleaning. There is either some leftover lubricant from the manufacturing process or deposits from the plastic case off-gassing. Both of these are almost invisible to the naked eye but make life harder for the optical drive.
