The optical drive can not correctly read some spots on the disc. You can try cleaning the disc again. You can replace the disc. Or try the disc in a different optical drive. Have you tried gently cleaning the disc? Those errors show that there's some spots on the disc that the optical drive is having a hard time correctly reading.

I'm not aware of a 12.7mm -> 9.5mm adapter, but I am aware of 5.25 adapters.

I tried to rip my new Charlie the Unicorn bluray, but MakeMKV was not able to process the audio tracks. I was able to rip it properly with HandBrake. It marked the audio channel as "Unknown (BD LPCM, 2.0 ch, 1536 kbps)" - might just be a bad encoding. blu-ray.com suggests that Charlie the...

The keyDB file needs to go in the folder that's configured as the 'MakeMKV data directory'. You can find what it is set to by looking on the General tab of MakeMKV's preferences.
