MKV is a container. It can contain lots of different things. When MakeMKV is used to rip a blu-ray, most commonly the resulting .mkv file contains h.264 encoded video ; a direct copy of what's on the disc. Physically, no. I do have a couple of LG based drives which get confused from time to time and need to be power cycled. My Pioneer BDR-S13UBK is probably my best drive. It is in a Vantec enclosure and connected via USB. Since I have multiple drives sitting on my desk, I prefer a tray loading drive.

There are different DVD Opening modes. Maybe MakeMKV is getting confused with the default opening mode.

It is my understanding that MakeMKV uses the hashes to make sure that what's read from the optical disc is correct. This is probably the most likely source of error. Decrypting the files, combining them and writing them out to disk is unlikely to be problematic.

Probably the optical drive is having trouble reading the disc.
The standard advice is to gently clean the disc and try again.
If the drive is powered by USB, it might not be getting enough power. Make sure both USB plugs are plugged into high powered ports.
