if the file discatt.dat was created, then you have not checked to decrypt the disc while ripping.

There is a checkbox for that when saving the backup of the disc.

Maybe another DVD Opening Mode can help.

Did you select the MVC track as well during ripping?

You can try another DVD opening mode and see, if that helps.

Did you make a decrypted backup? Because I can still see the AACS directory

It also could be RipGuard or ARccOS where there are intentional bad sectors on the disc itself.

Does the MakeMKV software automatically save cover art within the .mkv file? For blu rays I believe I see that "cover art" is listed amongst the different titles you can choose to rip or not rip. However I have not seen that listed for DVDs. So is cover art automatically saved within the ...

Hmm not sure if it can be a Mac thing, knowing their security settings can be quite difficult. Do you have a way of putting the BDMV and CERTIFICATE directory into a ISO and try opening the ISO?
