I’ve tried three times, cleaning again each time. In all cases it gets to the same spot, 41.5GB into it, which is about 2/3rds, and then has read errors.

Same here. I have cleaned this disk to the point of wearing it thin. Plays perfectly in the Oppo, but simply will not be seen by the drive when trying to rip.
