It must be a bug. I see the exact same failure with multiple discs on different computers, different drives, and different versions of MakeMKV. The digits after the ":" are identical "121266097". This is what I get for Spider-Man: DEBUG: Code 0 at r79-F\`Qopn&7y,}?R%):1212660...

I believe I'm also seeing a problem related to RipGuard. In my case, RipGuard seems to be preventing MakeMKV from making a Backup from DVD. It gets very near the end (within a few hundred kB of the total claimed, which is about double what it should be) after MANY RipGuard notices before failing. I ...
