unrelated to your problem, but why would you bother decrypting to an ISO image first? Just leave AnyDVD-HD running in the background and let MakeMKV read directly from the disc. update: issue still present in MakeMKV 1.4 beta.

additionally, the first chapter marker (which is not at 00:00:00) is being labeled 'chapter 0' instead of 'chapter 1'

BD+ is a second layer of security in addition to AACS encryption. It's not new, as it has been used since the fall of 2007. Best way to bypass it is to use AnyDVD-HD, then run MakeMKV as usual

already asked and answered, and the answer is no. There are plenty of other programs which can recompress the MKV file created by MakeMKV, such as mencoder.
