It was effectively a problem with the CD reader. I did not use it for years. I could not open it normally with the "eject" keypress. But when i opened it after a reboot, makemkv worked normally. I think I will remove it (mac pro early 2008)

If the drive is open, MakeMKV should detect that it is empty, and not spend a lot of time trying to read anything from it. If it does, the operating system may be telling it, "Yeah, there's a disk here." When was the last time you restarted the computer, in case the OS is confused? I did ...

cos' makemkv begins with searching cdrom reader, and opening file is greyed.
