For me, with English selected as my default language, it selects the core (DTS, DD) but not TrueHD or DTS-HD / Atmos.

Finally got around to trying this again. Re-downloaded v1.10.4. With MakeMKV NOT RUNNING, I ran the installer. It gave the error I reported in my original post. There was no prompt to uninstall the software. I tried several times. So I deleted its folder in program files (x86) and re-ran the install...

You would think that MakeMKV would not care, but, apparently, Windows does, because MakeMKV asks on start up, "What optical drives are there?", and relies upon the operating system to respond with a proper list. I do know that when I use remote desktop to control my main computer remotely...

I just noticed that a series from DVD I just ripped (Orphan Black [highly recommended!]) also exhibits the missing default flag. So, the missing flag was interpreted as Yes and it shows subs unless I manually turn them off. I don't do many DVD-based TV series, so maybe this is actually a common prob...
