I believe the correct source file name is 00254.mpls. A file named "...t114.mkv" is produced and by checking the the chapter duration and sequence in TSMuxer there's agreement with the original disc.

My disc does not have a "00920.mpls". I arbitrarily selected "00058.mpls" and surprisingly the resulting mkv shows everything in the proper order.
