I also think this may be an eac3to issue.

I own the Avatar Extended Collectors Edition Bluray, and I can confirm, that the extracted forced sub (PGS or .sup) stream from the mkv file created by Makemkv, when extracted with mkvextract, is handled without problems by BDSup2Sub.

I'm not sure but It's possibly because you fed BDSup2Sub a Forced Only PGS track to start with, so it doesn't need to apply the filter.

I believe HandBrake currently ignores Bluray PGS subtitles. I extract them and convert them to vobsubs then mux them into the HB generated mkv. The problem is that it's INTERLACED VC1.

As long as the bluray drive can write to some kind of media (cd-r, dvd+-r) then it should work ripping bluray disks with Makemkv on OSX. The drive doesn't have to be able to write bluray disks.
