I just gave this a shot today and the first thing I tried to rip was a really old DVD (Stargate SG1 Season 1 Disc 5). It failed to open the disc entirely, reporting a SCSI error, and then the drive locked up and I was unable to eject the disc. I removed the V6 bin from the data tar file and rebooted...

If this is some issue with the original master for the collection you're trying to rip from, then I'd try a different edition. Try a UK one, for example, 25 bucks for 1-4 from zavvi.

Another way that is somewhat of a pain, but very reliable, is to search the subtitles for text from the episode, which you can almost always find in "quotes" on IMDB. To do this you need to first extract the subtitles, using something like MKVExtractGUI https://sourceforge.net/projects/mkv...
