It is quite rare for me to find a TV show that doesn't already have separate files for each episode. I can only think of one case where I had to split by chapter, and I don't even remember what that was now. Out of curiosity, what TV show(s) are you trying to convert?

I've found that while tracks are often out of order, the mpls files are. For example, I have a disc of a particular TV series that I'm ripping now. For Disc 1, the playlists are 800.mpls, 801.mpls, and 802.mpls. However, they are track 5 (Game_Of_Thrones_Season_6_Disc_1_t05.mkv), 1, and 2. The vario...

Uncheck All works only at the level of the click. If you go into an episode, you can right-click and "Uncheck all" again at that level to remove the marks you highlighted.

On the "out of order" front, check the Source File name field in the Info section - usually, the number in that name will increase as you find each episode. If they're out-of-order in the list, if you name them IN order, you will usually be right. Frequently but not 100% at least in my ex...

The most frequent reason I've encountered for duplicate titles is language tracks. In every case, both versions of the same file will include English. It's the secondary languages that are different. If you browse the blu-ray disc itself, you'll probably find the jpeg is a background image for a menu or something. It's not a copy of the cover picture... that would make too much sense.
