it is probably as you say a 'bad rip', or non-compliant, or something using non-standard method. i can't see any info regarding how it was created, but i do see that i should be able to burn the files to double-layer dvd and playback on AVCHD player like PS3... so, kinda strange processing was done ...

i have gone to the directory just above where the index.bdmv file and the STREAM directory, etc. are and issued 'makemkvcon info file:.' and then ' makemkvcon mkv file:. all /tmp/blabla':
$ makemkvcon info file:.

hi. thanks for bringing this application to the open source community. i will share my first experience with it, attempting to convert a blu-ray disc structure on my hard disk. the problems i have may be due to the blu-ray files and not makemkv.
