Add "12 Monkeys" (Season 1, Disc 1) to the list.
For the German Release of the "Complete Series" it's the whole Season 1 (not just Disc 1). S01E01 was fine, after that every Episode has a chapter 0 with those color bars and a chapter 7 with "textless material".
