Update: just for grins I connected the drive directly to the rear USB ports of my motherboard and was able to rip without errors. I'm still going to order the external power adapter though.

I'm completely convinced that if you're using a Pioneer UHD drive that you should pump at least 30 watts to it, which is probably double the amount of power a old USB 3.0 port can supply. I gave it a proper cleaning and it's still throwing the error message. I tried another disc altogether and it gave the same error message. It seems like the drive doesn't get enough power over USB for sustained reads. I had it connected to the front USB of my case, so it should be getting enough ...
